The Effect of Active Pulmonary Physiotherapy on Postoperative Pulmonary Complication

Abstract
We carried out active the pulmonary physiotherapy to reduce the postoperative pulmonary complications. The active pulmonary physiotherapy included breathing exercise(chest wall expansion exercise and diaphragmatic breathing exercise) and
active-assistive coughing exercise and postural drainage with mechanical stimulation. We analyzed the clinical data from 200 patient with received conventional pulmonary physiotherapy(Group I) and 100 patient who received active pulmonary
physiotherapy(Group II) to evaluate effectiveness of active pulmonary physiotherapy in patients who were undergone upper abdominal operation, particularly in high risk patients on spirometry among patient over 60 years old.
@ES The results were as follows.
@EN 1) Total postoperative pulmonary complication rate in patients who received upper abdominal operation under general anesthesia was 12%(36/300).
2) Postoperative pulmonary complication rate in group I patients was 13.5%(17/200) and 9%(9/100) in group II patients.
3) Among pulmonary complications, pleural effusion occured most frequently(16 case) and atelectasis(13 case) and pneumonia(3 case) were occured in order.
4) Pulmonary complications were more frequently developed in pediatric patient(20.0%) and geriatric patient over 60 years old(14.3%).
5) Pulmonary complications were more frequently developed in pancreas surgery(18.2%) and liver surgery(17.4%), spleen surgery(16.6%), biliary tract surgery(13.8%) in order.
6) Pulmonary complication rate in high risk patient on spirometry were 40% in patient received conventional pulmonary physiotherapy and 20% in patient receieved active pulmonary physiotherapy.
7) Pulmonary complication rate in normal pulmonary function on spirometry were 7.6% in group 1 and 6.6% group II.
8) Postoperative pulmonary complication were developed 33.3% in high risk group among patients over 60 years and 7.3% nonrisk group among patients over 60 years.
As a conclusion, the active pulmonary physiotherapy method reduced the postoperative pulmonary complication. Therefore we think the active pulmonary physiotherapy method is a was good method for reducing the postoperative pulmonary complication.
